Most RPM programs monitor. Few activate.
A blood pressure reading above threshold tells you the patient’s hypertension isn’t controlled. It doesn’t tell you why they stopped taking their medication, whether they can afford the refill, or whether they understand why the medication matters. Without a human who can have that conversation, investigate the root cause, and resolve the barrier, the reading is just a data point. The patient’s condition continues to deteriorate, and the next reading is worse.
